Counting DOUBLE???

Any idea why the system would count some clicks TWICE, and others only once? At first I thought it was just counting the first click to each account twice, then all others once... but now it is counting some of the first clicks once, and some twice. Same with subsequent clicks.

Any ideas or suggestions would be greatly appreciated!
